You can take a bus from Bangkok to Chiang Mai spend a few days there exploring, then come back down to Bangkok and there are overnight bus/ferry combination tickets available. You get off the bus at Surat Thani and take a ferry to Koh Phagnan from there – this is a common route. Some great tips and advice for the authors, very in depth as well!
